Medical Records and Patient Complaint Information

In accordance with Texas Health and Safety Code Section 181.105, the following information explains how patients may request copies of their health care records, contact our licensing authority, and file a consumer or medical privacy complaint.

How to Request Your Medical Records

You have the right to request access to or obtain a copy of your health care records maintained by our office.

To request your records:

  1. Submit a written request for your medical records to our office. You may request a Medical Records Release/Authorization Form from our office or submit your own written request.

  2. Include enough information for us to identify the records you are requesting, including:

    • Patient name

    • Date of birth

    • Current contact information

    • The records or dates of service requested

    • Where you would like the records sent

    • Your signature and the date of the request

  3. Submit your request using one of the following methods:

  1. If additional information or authorization is needed to process your request, our office will contact you.

  2. Applicable copying, mailing, or other permitted fees may apply. We will notify you if a fee is required before processing the request.

How to Contact Our Licensing Authority

Chiropractors practicing in Texas are regulated by the Texas Board of Chiropractic Examiners (TBCE).

If you have a concern regarding the professional conduct or chiropractic services provided by a Texas chiropractor, you may contact the Texas Board of Chiropractic Examiners.

To file a complaint with the Texas Board of Chiropractic Examiners:

  1. Go to the Texas Board of Chiropractic Examiners Complaint Portal.

  2. Follow the instructions provided by the Board.

  3. Provide the requested information regarding the chiropractor and the circumstances of your complaint.

  4. Submit any supporting documentation requested or relevant to the complaint.

How to File a Texas Medical Privacy or Consumer Complaint

If you believe your protected health information or medical records have been improperly used, disclosed, collected, or handled, you may file a complaint with the Texas Attorney General Consumer Protection Division.

To file a complaint with the Texas Attorney General:

  1. Visit the Texas Attorney General's Consumer Protection complaint website.

  2. Select the complaint category that applies to your concern. For concerns regarding medical information, protected health information, or privacy, select the applicable privacy or medical-privacy complaint option.

  3. Provide the requested information regarding the health care provider or business involved.

  4. Describe what occurred and include relevant dates and circumstances.

  5. Upload supporting documents, if appropriate.

  6. Submit the complaint through the Attorney General's online complaint system.

  7. Retain the confirmation email and complaint reference number you receive after submission.

Complaints concerning a Texas chiropractor may also be submitted to the Texas Board of Chiropractic Examiners as described above.

Federal Medical Privacy Complaints

Patients may also file certain medical-privacy complaints with the U.S. Department of Health and Human Services Office for Civil Rights (OCR).

Information about filing a federal HIPAA complaint is available through the U.S. Department of Health and Human Services Office for Civil Rights.

This information is provided to explain the procedures available to patients for obtaining medical records and submitting complaints. Nothing on this page limits any rights available to a patient under state or federal law.
